{"id":4309,"date":"2015-09-07T08:51:21","date_gmt":"2015-09-07T12:51:21","guid":{"rendered":"http:\/\/www.khpi.org\/blog\/?p=4309"},"modified":"2017-02-13T14:05:14","modified_gmt":"2017-02-13T19:05:14","slug":"highest-prescribers-of-opioid-analgesics-in-kentucky-indiana-by-specialty","status":"publish","type":"post","link":"http:\/\/www.khpi.org\/blog\/highest-prescribers-of-opioid-analgesics-in-kentucky-indiana-by-specialty\/","title":{"rendered":"Highest Prescribers of Opioid Analgesics in Kentucky &#038; Indiana by Specialty."},"content":{"rendered":"<p><strong>Is there any\u00a0rational justification?<\/strong><\/p>\n<p><strong>Summary<\/strong>.<br \/>\nThis article in\u00a0a series further examines the prescription of opioid analgesics in Kentucky and Indiana, I identify the individual prescribers in each of some 70 different Medicare-designated specialties in both states who prescribed the highest number of opioid prescriptions. The differences within individual specialties, between specialties, and between the two states are staggering and beggar any obvious explanation. Single or small numbers of prescribers are outliers that shift the averages of their specialties significantly.\u00a0 Even within their own peer group, some pain-management providers are far-outliers. Some cities seem to have more than their share of highest prescribers. Cancer and designated hospice providers to not appear to be big users of opioids in this data. Much \u201cpain-management\u201d probably occurs within specialties traditionally considered as generalist. Generalist specialties should not be considered differently than pain-management specialties in public health and law enforcement efforts to deal with prescription drug abuse and diversion.<!--more--><\/p>\n<p>One of the most obvious observations made upon inspection of the 2013 Medicare Part-D prescription database is the extraordinarily wide range in the rate of prescription of virtually all drugs.\u00a0 The most common explanation for the bulk of such variation is simple preference rather than evidence-based medicine.\u00a0 The rate of prescribing of narcotic analgesics by individual providers is no exception to the rule.\u00a0 The chart below plots the number of prescriptions written by each of 2563 Kentucky generalists and 117 pain-management providers to Part-D Medicare beneficiaries in 2013. There is one dot for each individual prescriber. \u00a0[See<a href=\"http:\/\/www.khpi.org\/blog\/narcotic-analgesic-drug-use-by-medicare-patients-continued\/\" target=\"_blank\"> earlier articles<\/a> for a description and limitations of the Medicare database. The database identifies providers by name who prescribe a given drug to more than 10 Medicare patients.]<\/p>\n<p><img loading=\"lazy\" class=\"alignnone size-full wp-image-4310\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/gen-pain-ky-2013.jpg\" alt=\"gen-pain-ky-2013\" width=\"575\" height=\"737\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/gen-pain-ky-2013.jpg 575w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/gen-pain-ky-2013-117x150.jpg 117w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/gen-pain-ky-2013-234x300.jpg 234w\" sizes=\"(max-width: 575px) 85vw, 575px\" \/><\/p>\n<p>Altogether, providers in these six Kentucky specialties prescribed 1,126,110 prescriptions for opioids. The average number of Rx per provider was 420.\u00a0 However, the prescription rate was skewed heavily to the right such that the median (the middle number in a sorted list) was only 133.\u00a0 The prescription rates at the higher end of the curve ranged from very high to astronomical! The top prescriber was a Lexington anesthesiologist who wrote 13,382 prescriptions for\u00a0this group of Medicare patients. Statistically, that puts this physician 15 standard deviations above the overall average. Two standard deviations out is all that is needed to reach the customary p?.05 value of statistical significance ruling against chance alone in explaining outlier values.\u00a0 This in-the-next-universe value, and the other outliers on the chart have other explanations behind them that it behooves us to understand\u2013 wouldn\u2019t you think?\u00a0 We could be looking at great medicine or something else altogether.<\/p>\n<p>One might anticipate that the curves for the pain specialties (Anesthesiologist, Interventional Pain Management, Pain Management) would be different from that of the \u201cgeneralist\u201d specialties (Family Practice, Internal Medicine, Nurse Practitioner).\u00a0 Indeed, the median number of prescriptions for pain-management is 943 compared to 128 for the generalists. \u00a0In addition, a much greater proportion of pain-management providers write more than 1000 prescriptions, a useful break-point for comparisons.<br \/>\n<a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-ky-2013.jpg\"><img loading=\"lazy\" class=\"alignnone wp-image-4312\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-ky-2013-219x300.jpg\" alt=\"general-ky-2013\" width=\"270\" height=\"370\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-ky-2013-219x300.jpg 219w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-ky-2013-109x150.jpg 109w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-ky-2013.jpg 554w\" sizes=\"(max-width: 270px) 85vw, 270px\" \/><\/a>\u00a0<a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-ky-2013.jpg\"><img loading=\"lazy\" class=\"alignnone wp-image-4314\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-ky-2013-214x300.jpg\" alt=\"pain-ky-2013\" width=\"270\" height=\"379\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-ky-2013-214x300.jpg 214w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-ky-2013-107x150.jpg 107w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-ky-2013.jpg 521w\" sizes=\"(max-width: 270px) 85vw, 270px\" \/><\/a><\/p>\n<p>Nonetheless, the curves for the two groups share a basic shape: a heavy skew to the right with some providers hitting out of the ballpark.\u00a0 For example, a Family Practice physician from Lexington prescribing 7596 times to these Medicare patients lies 13 standard deviations above the average for generalists. \u00a0In fact, only 6 <u>pain-management<\/u> providers prescribe at a higher rate.\u00a0 Looking at the pain-management providers alone, and making an assumption that this is the peer-group against which one should compare prescription rates, the highest pain-prescriber is still 4 standard deviations above the mean of the same specialties. \u00a0On the other side of the coin and equally striking is the fact that 50% of the designated pain-management providers prescribe <u>fewer<\/u> than 1000 prescriptions for opioids!\u00a0 What are they doing differently from the other 50%?\u00a0 What might we learn from examining the difference?<\/p>\n<p>There is no doubt that Medicare\u2019s classification of a given provider\u2019s \u201cspecialty\u201d will be erroneous in some unknown number of cases. \u00a0On the other hand, many self-designated generalists emphasize pain management or otherwise prescribe large amounts of narcotics in their practices. After all, there are only 117 providers in the database that Medicare places in the pain-management specialties, fewer than I had guessed. The presence of high-prescribers in both groups and the shear number of prescriptions written by the generalists as opposed to pain-managers (884,257 \u00a0vs. 241,853) dictates that attempts to understand and control the overuse and diversion of opioids should approach both clusters of specialties in the same way. Pain doctors need not feel picked on!<\/p>\n<p><strong>Comparison with Indiana.<br \/>\n<\/strong>In earlier articles, I detailed the rate of <a href=\"http:\/\/www.khpi.org\/blog\/prescription-of-narcotic-analgesics-in-kentucky-and-indiana-a-comparison-2\/\" target=\"_blank\">narcotic prescription in Indiana,<\/a> and in particular in small towns surrounding the narcotic-, HIV-, and Hepatitis-C plagued town of Austin, Indiana.\u00a0 I do not believe it is coincidental that the Indiana hotspots for the prescription of Opana (the Austin drug of choice) are within 40 miles of that town. \u00a0Is there any conceivable reason why the small towns of New Albany and Seymour, IN should be third and fourth only to Fort Wayne and Indianapolis in the number of prescriptions of Opana written?<\/p>\n<p>Here are the prescribing profiles for the generalists and pain-management providers of Indiana as defined above for Kentucky.\u00a0 The shapes of the curves are very similar. The most striking difference is that Kentucky pain-managers prescribe more than Indiana&#8217;s. Note the difference in the vertical scale in the charts of the two state\u2019s pain-managers: Kentucky\u2019s is double that of Indiana. \u00a0I will also place a table of descriptive statistics comparing the two groups of providers in both states.<\/p>\n<p><a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013.jpg\"><img loading=\"lazy\" class=\"alignnone size-medium wp-image-4311\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013-219x300.jpg\" alt=\"general-in-2013\" width=\"219\" height=\"300\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013-219x300.jpg 219w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013-110x150.jpg 110w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013.jpg 528w\" sizes=\"(max-width: 219px) 85vw, 219px\" \/><\/a>\u00a0 \u00a0\u00a0<a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/general-in-2013.jpg\"><img loading=\"lazy\" class=\"alignnone size-medium wp-image-4313\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-in-2013-237x300.jpg\" alt=\"pain-in-2013\" width=\"237\" height=\"300\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-in-2013-237x300.jpg 237w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-in-2013-118x150.jpg 118w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/pain-in-2013.jpg 585w\" sizes=\"(max-width: 237px) 85vw, 237px\" \/><\/a><\/p>\n<p class=\"alignnone\"><strong>Where and how do the highest-prescribers practice?<a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-ky-in-2013.pdf\" target=\"_blank\"><img loading=\"lazy\" class=\"alignright size-thumbnail wp-image-4322\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-thumb-150x106.png\" alt=\"top-rx-thumb\" width=\"150\" height=\"106\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-thumb-150x106.png 150w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-thumb-300x213.png 300w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-thumb.png 337w\" sizes=\"(max-width: 150px) 85vw, 150px\" \/><\/a><\/strong><br \/>\nThere are high prescribers or far-outliers in many\u00a0of the specialties identified by Medicare.\u00a0 In the <a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-ky-in-2013.pdf\" target=\"_blank\">attached\u00a0table<\/a>, I identify the highest-prescribing individual in each major specialty. \u00a0Highlighted in green are the ten highest prescription rates. Pain-management providers had the highest rates of all, but there were very active opioid\u00a0prescribers in several other specialties. \u00a0No cancer or end-of-life provider was anywhere near the top\u00a0of this list. I wonder if healthcare providers in these specialties and cities would be able to predict who their\u00a0highest prescribers are?<\/p>\n<p class=\"alignnone\"><strong>A few cities had more of their share of highest-prescribers.<\/strong><br \/>\n<img loading=\"lazy\" class=\"alignnone size-full wp-image-4321\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/cities-most-hp-2013.png\" alt=\"cities-most-hp-2013\" width=\"421\" height=\"318\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/cities-most-hp-2013.png 421w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/cities-most-hp-2013-150x113.png 150w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/cities-most-hp-2013-300x227.png 300w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/cities-most-hp-2013-397x300.png 397w\" sizes=\"(max-width: 421px) 85vw, 421px\" \/><br \/>\nLexington is home to the highest\u00a0provider of 17 different specialties and Louisville to 15. Recall that there is only one &#8220;highest-provider&#8221;\u00a0per specialty.<\/p>\n<p><strong>Highest-Prescribers in Kentucky and Indiana compared.<\/strong><strong><a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in.png\"><img loading=\"lazy\" class=\"alignnone size-full wp-image-4319\" src=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in.png\" alt=\"largest-dif-ky-in\" width=\"601\" height=\"297\" srcset=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in.png 601w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in-150x74.png 150w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in-300x148.png 300w, http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/largest-dif-ky-in-500x247.png 500w\" sizes=\"(max-width: 709px) 85vw, (max-width: 909px) 67vw, (max-width: 984px) 61vw, (max-width: 1362px) 45vw, 600px\" \/><\/a><\/strong>To explore another measure of practice variation, I calculated the difference in prescription rates between the top prescribers in each of 64 specialties in Kentucky and Indiana.\u00a0 <a href=\"http:\/\/www.khpi.org\/blog\/wp-content\/uploads\/2015\/09\/top-rx-ky-in-2013.pdf\" target=\"_blank\">These could be striking<\/a>, exceeding 1000 prescriptions in 11 specialties.\u00a0 Of these, the rates were higher in Kentucky for: Anesthesiology, Emergency Medicine, Family Practice, Interventional Pain Management, Pain Management, Neurosurgery, and Physical Medicine.\u00a0 The specialties in Indiana where rates were higher included: Certified Nurse Specialist, Internal Medicine, Nurse Practitioner, and Plastic &amp; Reconstructive Surgery. Inspection of individual provider-files suggests that outlier prescription rates by one or a handful of prescribers can provide an explanation.<\/p>\n<p><strong>Conclusion<\/strong>.<br \/>\nThis prescribing data can be sliced and diced <em>ad infinitum<\/em>, but the overwhelming immediate conclusion is that there is colossal variation in the way physicians and other providers prescribe opioid analgesics.
